אני מצהיר/ה כי אני לא שוהה כעת בהודו.

אני מצהיר/ה כי אין לי ויזה להודו בתוקף, ולא הגשתי בקשה כפולה לויזה להודו.

מצהיר/ה בזאת כי המידע שמסרתי למטיילים בהודו נכון ואמיתי, בנוסף הנני מצהיר/ה כי עברתי על הפרטים שהזנתי ובדקתי כי לא נפלה טעות אפילו בספרה אחת. ידוע לי כי במקרה והמידע שמסרתי ימצא שגוי או מזויף בכל שלב, אני אחראי לתביעה משפטית ואזרחית / גירוש / מניעה או כל פעולה אחרת כפי שתתאים על ידי ממשלת הודו או מטיילים בהודו.

אני מאשר/ת בזאת כי לא תהיה לי שום טענה אזרחית או משפטית כנגד מטיילים בהודו או גורם אחר הקשור למטיילים בהודו, ובנוסף אני מסיר/ה ממטיילים בהודו כל אחריות לבדיקת פרטי האישיים. במקרה של טעות באחד מהפרטים שהזנתי האחריות לנזקים שייגרמו לי במישרין או בעקיפין כתוצאה מכך תהיה עליי בלבד ללא שום טענות כלפי מטיילים בהודו.

ידוע לי כי במידה ואבקש מכל סיבה שהיא לבטל את הויזה אחוייב בעמלת ביטול על סך 100 ש"ח בגין דמי טיפול ועמלות סליקה.

אני מאשר/ת בזאת כי כל הפרטים שנשלחו אודותיי נכונים, יש לי דרכון בתוקף מינימום לחצי שנה מיום הנחיתה בהודו, ושני דפים ריקים בדרכון.

ידוע לי כי אי מענה במייל לאחר 24 שעות לכל בקשה מצוות מטיילים בהודו יגרום לביטול הויזה ע"י באופן מיידי וכספי יוחזר לי בניקוי עמלת שירות טיפול בסך 100 שקלים.

I declare that I do not have a valid Indian Visa and have not applied for a duplicate Indian Visa.

I hereby declare that the information I have provided to Travelers in India is true and correct, and I further declare that I have gone through the details I have entered and checked that there is no mistake in even a single digit. I am aware that in case the information provided by me is found to be incorrect or false at any stage, I am liable for legal and civil action/deportation/debarment or any other action as may be appropriate by the Government of India or Travelers in India.

I hereby confirm that I will not have any civil or legal claim against Travelers in India or any other entity associated with Travelers in India and I further release Travelers in India from any responsibility for checking the personal details. In the event of an error in any of the details I have entered, the responsibility for any damages that may be caused to me directly or indirectly as a result will be solely mine, without any claims against Travelers in India.

I am aware that if I request to cancel the visa for any reason, I will be charged a cancellation fee of 100 NIS for processing fees and clearance fees.

I hereby confirm that all the details sent about me are correct, I have a passport valid for a minimum of six months from the date of landing in India, and two blank pages in the passport.

I am aware that failure to respond to an email after 24 hours to any request from the Travelers in India team will result in the visa being canceled immediately by Travelers in India and my money will be refunded to me minus a processing service fee of 100 NIS.
